Educational Goals

 Upon course completion Surgeons will:

· Understand and have applied the concepts fundamental to laparoscopic suturing and knot tying; develop facility in solving suturing and knot tying challenges with a variety of techniques, equipment and approaches.

· Have performed on Real Tissue Models: myomectomy (focusing on defect closure)  and TLH or Sacrocolpopexy (focusing on procedure steps, cuff closure and resuspension suturing).  Attendees will also demonstrate contained tissue removal.

· Understand safe and effective use of electrosurgical energy.

 

Rationale for this course:

             This 2 day course is an “All hands-on All the time” manual training experience intended to transform a surgeon’s laparoscopic skill set: reliable laparoscopic suturing and intracorporeal knot-tying, safe and effective use of electrosurgical energy, the ability to dissect the retroperitoneum, and the ability to repair organ injury. Achieving  advanced surgical skills is a liberating experience and gives a laparoscopic surgeon confidence in more advanced laparoscopic challenges.

Course Highlights:

Focused on advanced skill development -proficiency in suturing, knot tying and advanced laparoscopic procedures: myomectomy, total laparoscopic hysterectomy, retroperitoneal dissection, organ injury and repair.

100% Active Hands-On - learn technique by doing. 1:1 Attendee to box trainer.

Biologic Tissue Surgical Simulation Models (Real Tissue Models™) - highly realistic practice on highest fidelity surgical simulation. 

Course Format:

The course is composed of two days of skills development in dry and wet labs.   Each attendee works individually on their own box trainer and monitor screen.  Precepting is accomplished through individual guidance as well as through projection of the preceptors monitor. 

Course Schedule/Curriculum

Saturday September 28th, 2024 -   Clark Lecture Hall - Mount Auburn Hospital

 8:00am—12:00pm Hands on Dry Lab - Introduction of goals, station assignment, equipment check, warm-up exercises to establish favorable ergonomics. Introduce geometric fundamentals of laparoscopic suturing, knot-tying and knot mechanics.  Skills will include needle introduction, needle handling and loading, manipulation and driving , suturing and intracorporeal knot-tying (flat knot) with and without needle.  Contralateral and ipsilateral approaches introduced.

1:00 pm - 5:00 pm  - Hands on Dry Lab: Mastery of skills – Review of knot tying fundamentals with further practice through simulation task assignment.  Introduction of cinch knot mechanics with applications to intracorporeal and extracorporeal knot -tying., loop ligature and barbed suture.  Ipsilateral  and contralateral suturing will be reinforced and practiced through task-based  assignment. 

 

Sunday September 29th,  2024 – Clark Lecture Hall -   Mount Auburn Hospital

8:00am - 12:00 pm—  Ipsilateral and contralateral suturing and knot-tying will be reinforced and  practiced with emphasis on ipsilateral operating. Repetitive supervised practice will constitute the focus of this morning.  Single-port suturing and knot-tying may be requested.

1:00 pm - 5:00 pm —  Real Tissue Model Lab:  Myomectomy, Sacrocolpopexy.  Participants will operate on Myomectomy, and/or Sacrocolpopexy models. Tissue extraction will be taught. Each model will also have other minor procedural tasks eg. ovarian cystectomy, tubal re-anastomosis.
